As the leader of the interprofessional team investigating Mr. Jones’s issue, my primary focus would be on analyzing the systems and processes involved in his care, rather than on individual performance. We can use a system-based approach to identify root causes and create effective solutions that will prevent future incidents.

It is important that all team members are involved in both the solution and analysis phases. It is important to ensure all viewpoints are taken into consideration and the solutions are practical and effective.
