Good morning, fellow nurses. Today, I’d like to discuss with you the anticipated changes in the practice of nursing, particularly in health care delivery. We nurses have an important role to play in providing quality care for patients and supporting the improvements of the healthcare system. In order to do so, it is important that we stay up-to-date on all the new developments, especially in terms of health care reform.

The continuum or continuity of care is an important concept we should understand. The seamless and coordinated delivery of services for health across various settings such as clinics, hospitals and community centers is what this term refers. The nurses and other healthcare professionals must collaborate to make sure that the patients get appropriate support, care, and care throughout their entire health journey. The continuity of care is linked with improved outcomes for patients and lower costs.

Accountable care organization is another concept we need to be familiarized with. ACOs are a collection of health providers including doctors, nurses and hospitals who come together to improve and coordinate patient care and lower costs. ACOs aim to promote the delivery of high-quality, cost-effective healthcare through coordinated care, patient-centeredness, and quality measurement.

Another concept we should be aware of is medical homes. Primary care practices provide patient-centered and comprehensive care in medical homes. In medical homes, the focus is on establishing strong bonds between nurses and patients. It’s important that the goal of personalized care is coordinated and accessible.

Finally, nurse-managed healthcare clinics are managed and run by nurses. The clinics provide basic healthcare to communities that are underserved, and especially to those with little access to medical care. These clinics, which are nurse-managed, aim to lower costs and improve healthcare outcomes.

Discussion:

It is in line with my research on health reform that the feedback provided by three of our nurse colleagues was. The anticipated changes in health care delivery are focused on improving patient outcomes, reducing costs, and promoting patient-centeredness. In order to fulfill the new roles of nurses, they will require more knowledge and skills. This is especially true for leadership and collaboration. The nurse-managed clinics are essential in providing health services for the underserved.
