To practice evaluating patient encounter notes to identify appropriate CPT codes for billing, I would begin by reading through the patient’s medical history. These details can be very helpful in determining what services were provided by a physician during a visit. Then, after gaining a better understanding of what kind of treatment was provided, I’d look into any procedures or tests that were performed and then assign the appropriate CPT codes based on the information. I also consider any diagnoses that were made at the time of the visit, and their respective codes.
After reviewing any medication prescribed and treatments given during your visit, you will assign codes. Before submitting any claims, I’ll document the follow-up services or examinations that were provided.
If I follow these steps every time I review an encounter note in order to determine CPT requirements, it will increase the accuracy of my billing. It also increases my chance of getting reimbursed by insurance companies on time.
