The Chronic Disease Coalition is one such organization. It is a nonprofit national group that offers support and advocacy to individuals with chronic illnesses. They offer information about managing different diseases and connect users with local resources.
American Diabetes Association is a great resource for those with diabetes. They provide education, advocacy and support. The ADA has a number of programs online that offer vital information about managing diabetes. They also have support groups, both online and offline that help individuals connect with others who suffer from the disease.
Many cities also have organizations that are specifically designed to help those with chronic illness, such as the Helping Others Manage Chronic Disease (HOMCI) organization. HOMCI also offers individual counseling to patients or family members who are seeking more support from doctors.
