Public health nursing practicum documentation Public Health Nursing Practicum Documentation.

The overall goal of the Practicum:

I want to learn more about public health nursing, and develop the skills necessary to be a public health nurse. My goal is to develop a better understanding of the public health nurse, her functions and role in improving health and wellbeing of families and communities. I want to know how to integrate nursing theories and concepts into public health practice.

S.M.A.R.T. Goals:

1. Specific: I want to improve my skills of assessment and evaluation in public health nursing by the end my practicum.

Measurable : I will use the community health assessment instrument to assess at least 10 patients during my first 2 weeks practicum. During the practicum I will take part in 2 or more community health projects.

I will develop my ability to assess and evaluate, and take on more challenging evaluations and assessments.

It is important to develop my skills in assessment and evaluation if I want to become a competent nurse who practices evidence-based public health.

Time-bound : I must complete all assessments and evaluations in the first two weeks of the practicum. At least two community health evaluation projects will be completed during my practicum.

2. Specific: I want to improve my communication and leadership skills as a public health nurse by the end of this practicum.

Measureable: During my practicum, I will be the leader in organizing and implementing a minimum of 1 program on health education for the local community. I will attend at least 2 community meetings to discuss health-related issues.

Achievable: In collaboration with my preceptor, I will develop my skills in leadership and communication by taking part in meetings and programs aimed at improving community health.

It is important to develop my communication and leadership skills to become a competent nurse in public health and to ensure that I am able to effectively communicate messages about public health to the local community.

I am bound by time: During my practicum, I will implement and organize the health education programs and attend two meetings with community stakeholders.

3. Specific: My goal is to enhance my knowledge of program evaluation and planning by the time I complete my practicum.

I will plan and implement at least one program in public health during my practicum. I will assist with the evaluation of two or more public health programs.

Achievable: Working closely with my mentor, I will find opportunities to enhance my knowledge and skills of program evaluation and planning. This includes taking responsibility for the planning and evaluation of public health programs.

Relevant: I must develop my program planning skills and abilities to evaluate programs in public health. This will help me become an effective nurse, and ensure that I am able to design and evaluate successful public health programmes.

I am bound by time: During my practicum, I will be involved in planning and implementing at least one public health program as well as assisting in the evaluation at least two public health programs.
