Change model based on evidence-based practices
For me to use this model for implementing an evidence-based change to my clinical practice, I first need to identify a problem that needs to be addressed or a place where new strategies could help. You could do this by researching best practices, or by analyzing patient data and identifying areas that can be improved. This will allow me to select, critique and use my own best judgment in order to come up with a solution.
Next, the change will be designed, planned, and then piloted on a smaller scale, before a larger rollout. This allows time to make any necessary adjustments in case of unforeseen complications. Afterwards , effective integration &sustaining changes requires ongoing monitoring &evaluation coupled with open communication between all stakeholders during each step of this process . Then, all efforts should be put into ensuring that patients get quality healthcare while adhering to established protocols.
By following these steps one can effectively implement changes based on EBP into their clinical practice environment which can ultimately lead to improved outcomes &better patient satisfaction.
