In the course of her physical exam, her blood-pressure was found to be 150/90 mmHg. Also her BMI measured 35 kg/m2, indicating that she is obese. Her heartbeat was normal and there were no murmurs, or other sounds during auscultation. Her breathing rate was 12 breathes per minute with bilaterally clear lungs and good air intake. The abdominal examination revealed that the patient had normal bowel movements and a soft, rounded abdomen with no palpable mass.
Based on my findings , I formulated diagnosis of Hypertension (HTN) & Overweight (OW). For HTN, I prescribed lifestyle modifications such as increasing activity level , reducing salt intake & monitoring blood pressure daily while OW required dietary interventions like reducing caloric consumption by eating smaller portions over more frequent meals while avoiding highly processed foods whenever possible .
This health assessment project has helped us to gain a better understanding of how a particular condition can affect an individual. We are now able to create treatment plans that suit their needs, improving our chances for success in the future when managing conditions such as HTN and OW.
