Based on my research, I have identified an organization called the Health Promotion Council (HPC) which provides health promotion &care services for individuals in Philadelphia. This non-profit offers a wide range of resources such as educational programs , community outreach initiatives , and access to affordable healthcare through its network of providers &partners .

Additionally, they provide assistance to people who may be at risk because of their poverty, or due to other social determinants, by providing valuable resources, such as job and housing opportunities, food pantries. Additionally , they work closely with local stakeholders including schools, businesses, faith-based organizations &government agencies to ensure that all individuals can benefit from their services.

HPC Philadelphia offers a wide range of health-related services and programs. By utilizing its various programs &platforms one can gain access to quality healthcare while simultaneously receiving support for any underlying social issues that may be present.
