In my own clinical setting I apply this theory by developing individualized plans that are tailored to meet each patient’s specific needs. If I’m seeing a diabetic patient, I create an action program that will help him/her gain control of the disease by showing them how to measure their blood glucose at home and what food they should choose when dining out. In addition, when someone is recuperating from an accident or surgery, I provide them with instructions for safe activities they can do at home, such as using assistive devices or stretching exercises.
Overall, the goal is always to ensure that patients have all the necessary knowledge and resources available so that they’re able to take better control over their own healthcare which ultimately leads towards better outcomes in the long run.
